2.Your full contact information, full legal name, mailing address, telephone number, email address;

3.A statement confirming that you have a good-faith belief that the disputed use of the material is not authorized by the intellectual property owner, its representative, or applicable law;

4.A statement, made under penalty of perjury, that the information contained in your notice is accurate and that you are the intellectual property owner or authorized to act on behalf of the owner;

5.Your physical or electronic signature, or the signature of a person authorized to act on behalf of the rights holder.

B. Review and Removal Process

Upon receipt of a complete and valid infringement notice, Football Jersey House may remove, restrict access to, or otherwise disable the allegedly infringing content while the matter is reviewed. We reserve the right to take any additional action deemed appropriate to protect intellectual property rights and maintain compliance with applicable laws.

C. Counter-Notification Procedure

If you believe that content removed or disabled as a result of an infringement claim was removed in error, misidentified, or otherwise lawfully posted, you may submit a counter-notification containing the following information:

Identification of the material that was removed or disabled, including its location on the website before removal.
A statement, made under penalty of perjury, that you have a good-faith belief the material was removed or disabled as a result of mistake or misidentification.
Your full contact information, including: full legal name, mailing address, telephone number, email address, your physical or electronic signature.

D. Restoration of Content

Upon receipt of a valid counter-notification, Football Jersey House may provide a copy to the original complainant. Unless the complainant initiates legal proceedings seeking a court order to prevent restoration of the content, we may, at our discretion, restore the removed material or re-enable access to it within 10 to 14 business days following receipt of the counter-notification.

E. Misrepresentation and Liability

Please be advised that knowingly submitting false, misleading, or fraudulent infringement claims or counter-notifications may result in legal liability. Under applicable law, including Section 512(f) of the Digital Millennium Copyright Act (DMCA), any person who knowingly misrepresents that material or activity is infringing may be held liable for damages, costs, and attorneys' fees.
